Description
The Navajo Special Diabetes Program is seeking a qualified firm to provide comprehensive office‑furniture procurement, interior‑design layout, delivery, and installation services for the new Crownpoint Wellness Center facilities. The scope includes 2‑D/3‑D design drawings, supply of a full inventory of desks, chairs, tables, AV equipment, whiteboards, and ancillary items, as well as coordination of delivery and on‑site installation across three locations. Proposals must be submitted in sealed envelopes by August 31, 2026, with all required forms and certifications attached. The contract will require completion of delivery and installation by December 31, 2026, and the vendor must assume responsibility for any storage penalties incurred from delayed performance.
