The Department of Human Services is seeking a vendor to provide fish items to Danville State Hospital for the months of October, November, and December 2026. The vendor will be required to deliver the fish items to the hospital's dietary department. The contract will be for a period of three months. The vendor must comply with the special instructions and specifications outlined in the solicitation document, including grading, labeling, delivery time limitations, product condition, packaging, and contents. • The vendor must provide USDC Grade A fish items that meet the specified requirements. • The vendor must label the fish items with product identification, date of pack, name and location of packer or distributor, net weight, and ingredient statement. • The vendor must deliver the fish items within 90 days from the date of pack. • The vendor must maintain proper temperatures during transport and deliver the fish items in a frozen state. • The vendor must package the fish items in accordance with the specified requirements. • The vendor must provide samples for laboratory analysis and permit the facility to submit samples for testing.
