Post-Acute Care Facility Agreement

The University of Iowa is seeking a qualified supplier to provide post-acute care services through a collaborative model. The selected supplier will be expected to provide services such as skilled nursing and rehabilitation care, transitional care coordination, memory care and behavioral health support, medication management, and post-acute recovery. The University is looking for a supplier that can demonstrate the ability to provide these services and comply with all applicable state and federal regulations. The supplier will be required to submit a detailed proposal outlining their experience, qualifications, and approach to providing these services. Key aspects of the job include: • Providing post-acute care services through a collaborative model • Demonstrating the ability to provide skilled nursing and rehabilitation care, transitional care coordination, and other related services • Complying with all applicable state and federal regulations • Submitting a detailed proposal outlining experience, qualifications, and approach to providing services • Participating in a multiple-round bid process, including qualitative and financial evaluation components
