Key aspects of the project include:
• Scaling evidence-based falls prevention programs through the expansion of existing community capacity
• Leveraging healthcare and community care hub partnerships to deploy a falls prevention protocol
• Rapidly increasing access to screening, assessment, and interventions to reduce falls across five regions
• Using the CDC's Stopping Elderly Accidents, Deaths & Injuries (STEADI) Initiative tool to screen people for fall risk
• Implementing the Stepping On program, a multifactorial community-based falls prevention program
The project has a total expected funding of $4,781,733 over a 36-month period, with a start date of September 30, 2026.
