The Small Health Care Provider Quality Improvement Program supports the planning and implementation of quality improvement activities for rural primary care providers or providers of health care services. The program aims to increase access to high-quality health care services and improve health outcomes for patients in rural areas. Eligible applicants include domestic public or nonprofit private health care providers or providers of health care services, such as critical access hospitals, rural health clinics, or networks of small rural providers. The program will fund awards in four 12-month budget periods for a total four-year period of performance from 09/30/2026 to 09/29/2030.
• The program has a total available funding of $5,000,000 and expects to fund 20 awards with a funding range of $0 to $250,000.
• Eligible applicants must be located in a rural area and must not have previously received an award under this program for the same or similar project.
• The program requires applicants to implement evidence-based models or promising practices to demonstrate impact by the end of the four-year period of performance.
• The program has specific reporting requirements, including progress reports, annual performance reports, and program performance measures.
