This RFQ seeks a fully‑insured Medicare Advantage (MAP‑D) plan—both PPO and HMO options—to serve approximately 4,160 MNPS retirees and their eligible dependents. Vendors must submit detailed cost spreadsheets for three plan designs (baseline, baseline without the $150 medical deductible, and an optional enhanced design), provide historical claims and census data, and demonstrate the ability to maintain a broad provider network, meet CMS requirements, and deliver high‑quality retiree support services. The contract runs five years starting 1 Jan 2027 with annual renewal options, and the lowest monthly premium for the “no‑deductible” option will be the primary pricing evaluation factor. • Provide fully‑insured MAP‑D coverage (medical + Part D) for PPO and HMO plans covering retirees, spouses, families, survivors, and Medicare‑dependent beneficiaries. • Submit cost proposals via the Exhibit A cost spreadsheet (Option 1, Option 2, Option 3) and include a retiree‑advocacy service cost if applicable. • Deliver required data sets (member census, 2‑year medical claims, 1‑year pharmacy claims, provider utilization, risk scores) and comply with HIPAA BAA, insurance, and EGWP requirements. • Meet performance guarantees, network adequacy, and reporting obligations; optional vision integration may be submitted for review only.
