Description

The Alameda County Public Health Department (ACPHD) is seeking proposals from community-based organizations and accredited academic institutions to implement the Community Health Improvement Plan (CHIP). The CHIP aims to improve the health and well-being of Alameda County residents by addressing three priority areas: Access to Care, Promoting Economic Opportunity, and Promoting Peaceful Families and Communities. The selected organizations will work in partnership with ACPHD to implement existing programs and services that align with these priorities. The contract period will begin on October 1, 2026, and will be for an initial 12-month pilot, with the option to extend for an additional two years depending on available funding. A total of $2.0 million is available for this contract, and up to three awards will be made, one per CHIP priority area. • The Access to Care priority area focuses on linking populations to systems of care, with an emphasis on primary prevention. • The Promoting Economic Opportunity priority area focuses on advancing improved population health outcomes through education, internships, fellowships, residencies, or workforce pathways. • The Promoting Peaceful Families and Communities priority area focuses on delivering family-centered, early childhood programming and engaging caregivers, parents, and/or guardians as active partners and leaders.

Budget estimate

$500,000 – $1,000,000

The total available funding for this contract is $2.0 million, and up to three awards will be made. Assuming each award will receive a similar amount of funding, the estimated budget per award would be approximately $666,667. However, the actual budget may vary depending on the proposals received and the specific needs of the project. The budget may also be influenced by the complexity of the services required, the qualifications and experience of the bidders, and the level of competition for the contract.
