Description
Nyack Union Free School District is seeking a qualified vendor to provide temporary substitute nursing services (Licensed Practical Nurses and Registered Nurses) on an as‑needed basis to cover absences of district nurses, support field trips, and occasionally provide one‑to‑one student care. The contract runs from 1 July 2026 to 30 June 2027 with up‑to‑four optional one‑year extensions. Vendors must meet extensive credentialing, background, and insurance requirements, and submit a hard‑copy proposal package by 8 April 2026.
• Provide LPN and RN staffing for any district school site, ≈ 7‑hour school day per coverage event.
• Meet NYS nursing licensure, CPR/AED, fingerprint, background, OSHA, HIPAA, and other regulatory compliances for each staff member.
• Submit required exhibits (non‑collusion affidavit, insurance certificates, pricing sheet, etc.) and adhere to the district’s invoicing and payment procedures (Net 30).
