Request for Application (RFA) for 2026-2027 Diabetes Self-Management Education and Support (DSMES) Subawards

The Mississippi State Department of Health (MSDH) is seeking qualified applicants for the 2025-2027 Diabetes Self-Management Education and Support (DSMES) subawards. The goal is to enhance access to quality diabetes education and empower individuals with diabetes to effectively manage their condition. Up to four subgrantees will each be eligible to receive awards of up to $18,500 to establish or expand DSMES services. Key activities include program development and implementation, accreditation or recognition progress, staffing and training, participant engagement and education, data collection and reporting, and collaboration and outreach. • Establish or expand a DSMES program using an evidence-based curriculum • Deliver DSMES services through individual or group sessions • Pursue national recognition or accreditation through the American Diabetes Association (ADA) or the Association of Diabetes Care & Education Specialists (ADCES) • Collect and report key DSMES metrics • Participate in regular calls, technical assistance sessions, or learning collaboratives coordinated by MSDH
