COMMERCIAL PRINT ORDER - Teaching Your Teen to Drive

The Ohio Department of Public Safety is seeking a contractor to provide 50,000 copies of the "Teaching Your Teen to Drive" booklet. The booklet should be printed two-sided, head-to-head, 8 pages (4 originals) with a self-cover, in full color 4/4, on 8.5” x 11”, 70# white gloss text. The contractor should fold and saddle-stitch the booklets on the 8.5" side, shrinkwrap them at 25/pkg, and carton pack. The delivery address is 1583 Alum Creek Dr., Columbus OH 43209. The contractor should provide a delivery receipt with the following information: name of ordering agency, ODPS order number 47812, quantity ordered, quantity shipped, requested delivery date, actual date of delivery, title of material, and change order(s), if applicable. • The contractor should provide a proof of the booklet, including package and carton labels, to Ohio Department of Public Safety, Printing Services. • The contractor should use four-way (40 inches by 48 inches, cube form) pallets with four-way entry for delivery. • The loaded pallets should be in cube form, stackable, and not exceed a maximum weight of 5,000 pounds. • The contractor should send the invoice to 1970 W.
