Request for Proposals for Engineering Grant Services

The City of Blakely is seeking proposals from engineering firms to provide services for the implementation of a Community Development Block Grant (CDBG) project. The project involves making Water & Sewer System Improvements. The selected firm will be responsible for planning, design, and project follow-through. The city is looking for firms with experience in CDBG projects and a strong record of successfully assisting local governments. Key aspects of the proposal include: • History of the firm and resources • CDBG/EIP/RDF experience • Key personnel/qualifications • Current workload • Scope and level of service proposed • Experience with similar projects and references • Fees and/or percentages associated with the Preliminary Engineering Report (PAR) and Design and Construction Management Services • Errors and Omissions Insurance • Statement of Qualifications Form • Section 3 Certification Form (if applicable) • Unique Entity Identifier (UEI) number (if available)
