Community Based Outpatient Clinic - Yreka, CA

The Department of Veterans Affairs is seeking a proposal for a Community Based Outpatient Clinic (CBOC) in Yreka, California. The CBOC will provide primary care services to veterans, including health promotion, disease prevention, and management of chronic conditions. The contractor will be responsible for providing all personnel necessary to deliver contracted primary care services and support clinic operations. The staffing requirements include a physician director, primary care providers, registered nurses, licensed practical nurses, and administrative associates. Key aspects of the job include: • Providing comprehensive primary care services to veterans • Managing clinic workflow and ensuring patients are placed in examination rooms in a timely manner • Collaborating with PACT staff to develop comprehensive health care plans and care management plans for patients • Providing leadership to the team, including shared delegation of appropriate care and care processes to appropriate team members • Reviewing available clinical and performance data with the team and focusing on continuous improvement of critical team processes • Ensuring patients have same-day access for face-to-face and virtual care visits during regular clinic hours • Collaborating with PACT staff to develop personal health plans that incorporate care management and care coordination appropriate to the patient's needs
