Yuba County Transitional Housing Placement Plus Program

Yuba County is seeking proposals from qualified, licensed Foster Family Agencies (FFAs) to participate in the Transitional Housing Placement Plus (THP-Plus) Program. The program aims to provide a safe living environment and supportive services to former foster youth and former foster parenting youth, helping them achieve self-sufficiency and develop life skills. The selected contractor will be required to provide housing models, programs, and services, including case management, crisis intervention, and educational advocacy. The contract term is two years with two one-year extension options. • The program will serve former foster youth and former foster parenting youth who have emancipated from foster care, group home care, or juvenile probation. • The contractor will be required to provide a detailed plan for monitoring the placement of participants and a written participant agreement. • The contractor will also be required to provide a monetary allowance for each participant and a system for the payment of utilities, telephone, and rent. • The contractor will be expected to provide resources for participants to work toward their Transitional Independent Living Plan (TILP) goals. • The contractor will be required to provide access to individual and group therapy needs and appropriate medical care for participants. • The contractor will be expected to provide educational advocacy and support activities, including ensuring participants are enrolled in and attend a program to obtain a high school diploma, General Education Development (GED), College, Jr. College and/or vocational education.
