Description
Steuben County is seeking a quotation for on-site (mobile) industrial hearing evaluations with reports and summaries. The selected vendor will provide one day per year of on-site mobile industrial hearing evaluation services, including program oversight, reports, and summaries, and standard threshold shift review and reporting. The quote shall be for the evaluation of 120 to 150 people, with an additional per-person rate over 150 people. The location of on-site testing will be at the Steuben County Department of Public Works, Mt. Washington Shop (Station #3), 7029 County Route 113, Bath, New York 14810.
Key requirements include:
• Compliance with OSHA 29 CFR 1910.95 and Steuben County policy
• Establishment of baseline evaluation for employees within 1 year of employment
• Provision of written explanation of evaluation results to employees
• Comparison of annual evaluation results to verify test validity and determine the presence of a standard threshold shift
• Notification of Steuben County in the event of a standard threshold shift
• Maintenance of records, including employee name, date of audiogram, testing conductor's name, and date of most recent acoustic or exhaustive calibration
• Provision of testing results and other information in protected digital format
The contract will be for a one-year term, with the opportunity for up to four one-year renewals, upon mutual consent of both parties.
