Description
The Indian Health Service is seeking a contractor to relocate a laboratory door and pass-through specimen window at the Haskell Indian Health Center in Lawrence, Kansas. The contractor will be responsible for removing, replacing, and installing the door and window as shown on the drawing and statement of work. The period of performance for this requirement is 120 calendar days from Notice to Proceed.
Key requirements for the contractor include:
• Providing a firm, fixed price quote on the SF18 form and a completed attached bid sheet
• Submitting technical specifications of the statement of work and acceptable materials list
• Providing relevant past performance of this type of work
• Including a bid bond form (SF24) attached at $35,000
• Meeting all technical specifications and requirements outlined in the statement of work
