The vendor must submit the quote via email to the Procurement Analyst by the due date. The following administrative documents must be submitted with the quote: Bidder Declaration, Commercially Useful Function (CUF) Documentation, DVBE Declarations, Civil Rights Certification, Iran Contracting Act Certification, Payee Data Record, and Seller's Permit.
• The quote must be submitted on company letterhead and include company name, sales representative name, address, phone number, and email address.
• The quote must be valid for a minimum of 30 days.
• The vendor must comply with the instructions and requirements found in the solicitation.
• Failure to comply with any of the instructions or requirements may cause the response to be rejected.
