Description
This contract calls for a full‑time, agency‑based Clinical Case Manager to provide mental‑health assessment, case management, group co‑facilitation, and referral coordination for adjudicated youth within Cuyahoga County’s Juvenile Court system. The contractor must hold a relevant bachelor’s degree, maintain appropriate licensing, and deliver services on a per‑diem basis while adhering to Medicaid billing requirements and county‑provided HIPAA‑compliant facilities. The County supplies office space, training, and supervision, while the vendor supplies the qualified case manager and handles all documentation, billing, and performance reporting throughout the six‑month service period.
