HEALTH PROMOTION/DISEASE PREVENTION PROGRAMS FOR NAPA AND SOLANO COUNTIES

The County of Solano, Department of Health and Social Services, Napa/Solano Area Agency on Aging, is seeking a qualified proposer to provide Health Promotion and Disease Prevention Services to older adults. The primary responsibility of Health Promotion and Disease Prevention is to improve health outcomes and reduce chronic illness among older adults.
