Description
The Department of Veterans Affairs is seeking a contractor to provide nonsterile compounding preparation services for the Mann-Grandstaff VA Medical Center. The contractor will be responsible for receiving prescriptions, performing nonsterile compounding, and mailing the completed compounds directly to the Veterans. The contract will be for a period of 5 years, with an estimated annual quantity of 48 units of Magic Mouthwash and 5 units of Diltiazem 2% ointment. The contractor must be licensed in at least 3 states and follow USP 795 standards.
• The contractor will be required to provide nonsterile compounding preparation services, including Magic Mouthwash and Diltiazem 2% ointment.
• The contractor must be licensed in at least 3 states and follow USP 795 standards.
• The contractor will be responsible for mailing the completed compounds directly to the Veterans.
• The contract will be for a period of 5 years, with an estimated annual quantity of 48 units of Magic Mouthwash and 5 units of Diltiazem 2% ointment.
• The contractor must provide a guaranteed minimum of contract determined prior to award.
• The contractor will be required to submit invoices monthly, with a description of the items delivered, quantity, unit price, and extended price.
