Description
The Indian Health Service (IHS) is seeking a vendor to provide an ultrasound system, including a Sonosite LX Ultrasound System, a Transducer C5-1, and a Transducer L12-3. The system will be used at the Supai Clinic, and the vendor must be an authorized distributor of the proposed equipment. The quote must include all aspects of the RFQ, including equipment, services, and options. The government intends to award a purchase order to the responsive and responsible vendor whose conforming response is determined to provide the lowest price technically acceptable to the Government.
• The ultrasound system must be new, not remanufactured or "gray market" items.
• The vendor must be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor, or authorized reseller.
• The quote must include a copy of the authorized distributor letter from the manufacturer to verify the vendor's authorization.
• The system must be shipped FOB Destination, with shipping cost included in the vendor quote.
• The government will not split this requirement, and quotes must include all aspects of the RFQ.
