The Florida Agency for Health Care Administration is issuing a Request for Applications (RFA) to establish grant agreements with eligible subrecipients under Florida's Rural Health Transformation Program. The program aims to expand access to primary and preventive care in Florida's rural communities by funding the startup and initial operation of new or expanded rural clinics and satellite clinic sites. The initiative is classified as a standalone and startup initiative, with funding limited to the startup and early operational phase of new or expanded clinic sites.
• Key components of the program include startup and early operational funding, health information technology integration, workforce recruitment and training, care coordination, and technology and clinical equipment purchases.
• Eligible applicants include rural health clinics, federally qualified health centers, critical access hospitals, county health departments, and other healthcare organizations.
• Applications will be evaluated based on organizational capacity, program design and clinic model, rural access impact and site eligibility, physical plant and site compliance, technology and HIT plan, partnerships and regional integration, and sustainability and revenue plan.
