Description
The Office of State Procurement is seeking bids for the replacement of flooring and painting/sheetrock at the Louisiana Department of Veterans Affairs (DVA) located at the LA War Veterans Hospital. The job is to be performed in accordance with the enclosed scope of work and specifications. A mandatory jobsite visit is required prior to bid opening, and vendors must submit a completed Attachment B - Jobsite Verification Form or a signed letter from the Agency representative stating that the vendor has visited the jobsite. The contract may be extended for 2 additional 12-month periods at the same prices, terms, and conditions.
• The bidder must furnish all necessary labor, materials, tools, equipment, and supervision to complete the flooring replacement and painting/sheetrock.
• The bidder must comply with all applicable federal and state laws and regulations.
• The bidder must provide a bid bond in the amount of 5% of the bid price.
• The bidder must provide a performance bond if required.
• The bidder must deliver all items F.O.B. destination or as otherwise provided.
