Test Scoring Sheets

The City of Philadelphia is seeking bids for test scoring sheets. The sheets should be compatible with specific scantron models and have a particular format. The city requires 10 packages of 500 sheets each. The bid will be awarded to the lowest responsive and responsible bidder. The city may award the bid as a whole or by line item, depending on what is in the best interest of the city. • The test scoring sheets should have 200 multiple choice questions with a response format of a-e and be two-sided, 8 1/2" x 11". • The sheets should be compatible with scantron models 8000, 8200, 8400, 8600, scan mark 2000, 2250, and 2500, as well as ncs opscan series optical mark readers. • The city requires 10 packages of 500 sheets each. • The bid should include the unit cost for each item listed in the bid. • The city may require additional documentation, such as a consent and authorization form and an LGBTQ applicant opportunity data form.
