Description

The Administration for Community Living (ACL) will award up to seven cooperative agreements to Area Agencies on Aging (AAAs) or qualified community organizations to develop dementia‑capable Community Health Worker (CHW) programs across the National Aging Network. The goal is to embed CHWs in community‑based aging services to improve dementia screening, referrals, evidence‑based interventions, and overall quality of life for persons living with dementia and their caregivers. Funding per award ranges from $200,000 to $250,000, with total estimated funding of $1.75 million. Projects are expected to start July 1 2027 and run for approximately one year. • 7 cooperative agreements anticipated, each $200‑$250 k. • Target applicants: Area Agencies on Aging (AAA) or entities partnered with an AAA; foreign entities ineligible. • Scope: recruit, train, and integrate CHWs; develop dementia‑focused screening and referral pathways; deliver evidence‑based interventions; reduce care barriers. • Deliverables include a detailed implementation plan, performance metrics, and final evaluation report. • Funding instrument: Cooperative Agreement (grant‑type).

Budget estimate

$200,000 – $250,000

The forecast indicates total anticipated funding of $1,750,000 for seven awards, meaning each award will receive between $200,000 and $250,000. Assuming the full award ceiling is granted, a successful applicant could earn up to $250k for a roughly 12‑month project. The lower bound reflects the award floor of $200k. Factors influencing the final amount include the scope of services proposed, matching‑fund requirements, and any cost‑sharing commitments.
