Description

The Alzheimer's Disease Programs Initiative (ADPI) is a funding opportunity that aims to support and promote the development and expansion of dementia-capable home and community-based service (HCBS) systems in states and communities. The initiative has two application options: Grants to States (Option A) and Grants to Communities (Option B). Option A is designed for state agencies to create and sustain a dementia-capable HCBS system, while Option B is for community-based organizations to enhance and strengthen existing dementia-capable HCBS systems. The program focuses on providing quality, person-centered services and supports to individuals living with dementia and their caregivers, helping them remain independent and safe in their communities. The key objectives of the program include creating and expanding dementia-capable HCBS systems, ensuring access to comprehensive and sustainable services, and addressing specific service gaps in existing systems. • The program has two application options: Grants to States (Option A) and Grants to Communities (Option B) • Option A is designed for state agencies to create and sustain a dementia-capable HCBS system • Option B is for community-based organizations to enhance and strengthen existing dementia-capable HCBS systems • The program focuses on providing quality, person-centered services and supports to individuals living with dementia and their caregivers • The key objectives of the program include creating and expanding dementia-capable HCBS systems, ensuring access to comprehensive and sustainable services, and addressing specific service gaps in existing systems
