Infusion Therapy Consulting Services

The purpose of this project is to establish sustainable, outpatient clinic-based non-oncological infusion therapy services through a partnership between Central Health and CommUnityCare. This initiative will expand timely access to medically necessary treatments, improve care coordination across primary and specialty services, and reduce avoidable emergency department utilization for eligible patients. Key aspects of the project include: * Establishing clinic-based infusion services * Improving care coordination * Reducing wait times * Improving patient experience * Developing a financially viable care model The proposal should include a narrative response to the prompts, a completed acknowledgement of receipt form, a completed historically underutilized business form, and other supporting materials as necessary.
