The VA Portland Health Care System requires a one‑time production of 5,000 custom 9” × 11½” open‑end window envelopes with a gummed flap on the short side. The envelopes must be Kraft‑type (JCP V10), feature a 4½” × 1¼” transparent window positioned per specifications, and include black one‑sided printing. The contractor must perform full pre‑press checks, submit a press‑quality PDF proof for approval within two workdays, and then produce the envelopes, dividing them into 80 equal sub‑lots with random‑copy certification and GPO labeling. Finished cartons are to be shipped FOB destination to the VA’s Vancouver, WA facility, with a shipping notification email on the day of dispatch. All required GPO forms (905, 917, 2678) and a W‑9 must accompany the invoice.
