Description
The Indian Health Service is seeking a contractor to provide commercial dental supplies to the Zuni Comprehensive Community Health Center in Zuni, New Mexico. The contractor will be required to provide a wide range of dental supplies, including equipment and materials for routine, preventive, restorative, endodontic, prosthodontic, oral surgery, and other dental procedures. The contract will be for a base period of 12 months, with four option periods of 12 months each. The contractor will be required to deliver products within five business days of receipt of an order, and to provide pricing for all required contract line items. The government reserves the right to increase or decrease quantities based on actual program requirements.
• The contractor must provide products that are new, unused, commercially available, factory sealed, and free from defects.
• The contractor must provide products that are unexpired and have a minimum remaining shelf life of 12 months upon delivery.
• The contractor must deliver products that are suitable for their intended clinical use.
• The contractor must provide sufficient technical literature, manufacturer specifications, product data sheets, or other documentation to demonstrate that the proposed products meet the requirements of the solicitation.
