Accidental Death and Dismemberment Blanket Accident Insurance Florida Statutory Death Benefits

The Department of Management Services is seeking bids for accidental death and dismemberment blanket accident insurance with Florida statutory death benefits. The successful bidder will provide commodities and/or contractual services as described in Attachment A, Scope of Work. The term of the contract is specified in Attachment H, Draft Term Contract. The Department intends to make a single award, but reserves the right to make no award. The approximate annual spend on the expiring blanket accident insurance policy coverage is $130,000. • The bid must include a completed Attachment B, Mandatory Responsive Requirements, and Attachment C, Price Sheet. • The bidder must select an insurer with an AM Best Key Financial Rating of at least A-. • The bidder must provide the selected insurer's most recent AM Best Rating Information. • The bid must include a specimen policy or list of policy forms of the proposed coverage. • The bidder must complete and submit Attachment I, PUR 1355, Foreign County of Concern Attestation.
