IN-HOME SERVICES-COUNTY OF LANCASTER, PA

The County of Lancaster, Pennsylvania is seeking bids for a Request for Proposal for Providing In-Home Services for Consumers for the Lancaster County Office of Aging. The base contract will be for a period of three years with an option to extend the contract for two additional periods of one year each. The services include personal care, home support, and other in-home services for consumers aged 60 and over who are assessed and determined to be in need of these services by the Area Agency. The provider must be able to staff all hours of service authorized once the provider assignment has been finalized. The provider must also maintain a capacity to directly provide and/or arrange for the provision of services necessary to meet all the needs of consumers as defined in the Area Agency's service plan. • The provider must be able to provide services seven days a week for the hours and shifts designated in consumer service plans and service orders. • The provider must be able to respond to consumers' needs for care within one hour of a consumer's regularly scheduled service. • The provider must be able to provide backup service for all consumers. • The provider must be able to communicate with the Area Agency regarding the status of referrals for service, service cancellations, schedule changes, and other important updates. • The provider must be able to maintain confidentiality and adhere to ethics standards in providing services to consumers.
