Description
The Department of Veterans Affairs is seeking information from qualified sources to perform Siemens Uroskop De-Installation Services at Captain James A. Lovell Federal Health Care Center in North Chicago, IL. The services include safely deinstalling the equipment in accordance with OEM specifications, OSHA, FHCC Safety guideline, and FHCC Infection control standards. The contractor must provide detailed descriptions of deinstallation performed, including itemized descriptions of service performed and signatures of the Field Service Engineer (FSE) and a VA employee.
• The contractor must have an established business in the industry of removal and relocation of medical equipment.
• The contractor must have experience in performing services in accordance with OSHA, FHCC Safety guideline, and FHCC Infection control standards.
• The contractor must be licensed or capable of obtaining the necessary licensing required to perform these services within the State of Illinois.
• The contractor must provide a report indicating maintenance performed and pass or fail criteria upon completion of all work.
