The Agency for Health Care Administration (AHCA) is issuing this Request for Applications (RFA) to establish grant agreements with eligible subrecipients under Florida's Rural Health Transformation Program (RHTP). The RHTP is a five-year, federally funded initiative administered by AHCA under the Centers for Medicare and Medicaid Services (CMS) Cooperative Agreement. Key components of the program include VBP infrastructure development, population health and data infrastructure, care coordination infrastructure, and interoperability and HIE integration.
