Aftercare Program Services

The County of San Mateo's Human Services Agency is seeking a provider for Aftercare program support services. The program aims to provide case management and referral services to community-based providers for eligible foster youth. The services will include participant outreach, targeted workshops, and case management to help participants achieve self-sufficiency and lifelong connections. The provider will be required to collect and store participant data, enter data into a County-provided system, and provide annual survey results. The contract term is anticipated to be 36 months, with an option to extend for an additional 24 months. • The provider will serve a minimum of 35 participants per year. • The provider will conduct targeted outreach to Transitional Housing Placement Plus (THP-Plus) participants and other eligible youths. • The provider will participate in regular meetings with the County and provide updates on services provided to participants. • The provider will conduct targeted workshops once a quarter to THP+ participants and all other eligible youth around independent living skills. • The provider will participate in a 90-day transitional meeting with youth aging out of the AB12 program to discuss participant need for Aftercare services.
