The facility is located in Chinle, AZ, and the contractor will be required to provide healthcare services to the facility. • The contractor will be required to provide an OB/GYN Physician to work at the facility. • The facility is an Indian Small Business Economic Enterprise (ISBEE) Set-Aside. • The contractor will be required to provide a proposal that includes all necessary information, including an NPDB report. • The facility has a waitlist for on-campus housing, and the contractor will be required to book a hotel room at the Best Western hotel, as the pager will not work at other nearby hotels.
