The contractor will be required to provide a range of services, including clinical assessments, case planning, service linkage, and follow-up activities. The contractor will also be required to maintain accurate data on youth served, assessments completed, case plans developed, service referrals and enrollments, program completions, follow-up contacts, and outcomes.
Some key features of the program include:
* On-site clinicians and support staff
* A welcoming youth and family lounge
* Medi-Cal claiming for eligible services
* Warm handoffs to community youth service providers
* A focus on family-centered, trauma-informed, and culturally responsive services
* A commitment to reducing language, access, and service barriers for youth and families
* A goal of redirecting eligible youth from deeper justice system involvement toward positive development, educational engagement, and long-term well-being.
