The Cumberland County Public Health (CCPH) is funding a pilot to reduce chronic absenteeism among middle‑school students through supportive home‑visits. Two middle‑school‑level public schools in the Portland, Maine area may each receive up to $15,000 to design and implement a data‑driven, family‑centered home‑visit program aligned with the LEAP model. Awardees must form an implementation team, identify risk/protective factors, engage at least ten families, conduct a minimum of three home visits per family, and provide documentation of outcomes.
