Description
The County of Siskiyou is seeking a qualified provider to design, implement, and manage a multi‑service Wellness and Recovery Program for individuals with serious mental illness across the county. The contractor will deliver evidence‑based practices, extensive outreach, culturally competent services, peer‑support staffing, and full integration with the County’s electronic health‑record system, while providing monthly progress reports and meeting all insurance and compliance requirements.
• Deliver a menu of recovery‑oriented services (peer support, skill‑building classes, health education, substance‑use education, transportation, respite, etc.).
• Employ at least one full‑time equivalent Peer Support Specialist and ensure bilingual, culturally aware staffing.
• Implement and achieve routine use of the County‑mandated EHR within six months of contract start.
• Provide monthly written reports, maintain detailed participant records, and cooperate with County evaluations.
• Maintain required insurance coverage and adhere to all HIPAA, state, and local regulations throughout the three‑year contract term.
