Residential Behavioral Health Services - Low Acuity

The Dane County Department of Human Services is seeking proposals for the provision of residential behavioral health services to support clients in low-acuity residential care. The services will be provided in a Community-Based Residential Facility (CBRF) or an Adult Family Home (AFH) and will focus on stabilizing mental health symptoms, reducing psychiatric hospital readmissions, and supporting shorter lengths of stay in inpatient and other higher levels of care. The provider will be expected to provide a range of services, including assessment, treatment planning, counseling, and medication management. The contract will be for a period of one year, with the option to renew for up to four additional one-year periods. • The provider must be licensed by the Wisconsin Department of Health Services to operate a CBRF or AFH. • The provider must have experience in providing residential behavioral health services to adults with serious and persistent mental illness. • The provider must have a strong track record of delivering high-quality services and achieving positive outcomes for residents. • The provider must be able to demonstrate a commitment to providing culturally responsive and trauma-informed care. • The provider must be able to work collaboratively with other providers and stakeholders to ensure seamless transitions and coordination of care.
