LEADERSHIP ACTION MODEL (LAMS CARDS)

The US Government Publishing Office is seeking a contractor to produce 1200 Leadership Action Model (LAMS) cards. The cards will have a trim size of 3.125" x 2.25" and will be printed with a 4-color process. The contractor will be required to laminate the entire surface of the cards and die-cut all four corners. The contractor must also provide a 'Press Quality' PDF 'soft' proof for content only. The proof will be evaluated for text flow, image position, and color breaks. The contractor will be responsible for packaging the cards in quantities of 30 with shrink film and delivering them to the USCG TRACEN YORKTOWN - LAMS DETACHMENT. Some key requirements of the project include: • The contractor must provide a W-9 form before payment can be made. • The contractor must submit invoices within 30 days of job shipping/delivery. • The contractor must notify the ordering agency on the same day that the product ships/delivers via email. • The contractor must provide copies of all delivery, mailing, and shipping receipts upon agency request. • The contractor must use GPO form 905 to mark all shipping containers.
