Coordinated Street Outreach Program

The City of San Diego seeks one or more contractors to operate a Coordinated Street Outreach Program that delivers low‑barrier, Housing‑First outreach services to unsheltered individuals throughout the city. The contractor will conduct daily street visits, provide basic necessities, coordinate shelter intake, develop housing plans, and maintain real‑time data in the city’s Homelessness Management Information System (HMIS). The effort includes supporting CARE community events, integrating with the Coordinated Entry System (CES) and the 2‑1‑1 San Diego database, and delivering regular performance and outcome reports. The contract is initially for one year (through June 30 2027) with up to four optional one‑year extensions. • Perform daily outreach in assigned zones, building rapport and providing food, clothing, safety resources, and referrals. • Manage participant data entry (HMIS) within three days of service, coordinate shelter intake, and develop individualized Housing‑First plans. • Support CARE events (setup, intake, data collection), maintain a community‑engagement plan, and submit monthly/quarterly performance reports. • Provide an Emergency Preparedness Plan within 60 days of contract award and maintain compliance with city ordinances, TRUST surveillance technology mandates, and insurance/bond requirements. • Retain all contract records for five years and deliver a close‑out packet summarizing outcomes and leveraged resources.
