The Columbus Public Health is seeking licensed sewage treatment system installers to bid on the replacement of a sewage treatment system. The project involves the installation of a NORWECO GREEN 960 + AT 1500 UV OR EQUIVALENT NPDES SYSTEM. The estimated cost of the project is $20,000. The contractor must itemize all parts of the project in the bid, including materials, labor, equipment, etc. The project must be completed within 30 consecutive calendar days from the issuance of the 'Notice to Proceed' and in compliance with all applicable Columbus Public Health regulations and Ohio Administrative Code 3701-29. • The contractor must be approved or become approved by the 'Notice to proceed' to operate as a Registered Sewage Treatment System Service Provider with Columbus Public Health. • The contractor must provide proof of a surety bond of not less than twenty-five thousand dollars and submitted to the Ohio Department of Health. • The contractor must provide proof of testing requirements, proof of no less than five hundred thousand dollars general liability insurance coverage, proof of completion of six (6) continuing education hours, and applicable fees. • The contractor will be paid for the work after the project is complete to Columbus Public Health's satisfaction and complies with the Ohio Administrative Code 3701-29.
