Dawson County is seeking proposals from responsible firms to provide onsite medical services for a new clinic located in Dawsonville, GA. The services will include preventative care, advanced primary care, acute care, disease management, occupational health, and well-being services. The proposer must agree to the terms and business requirements outlined in the RFP, including maintaining a safe and compliant environment, administering a patient satisfaction survey, and providing an electronic medical record platform.
• Preventative care services such as annual exams, blood work, vaccines, screenings, and sports physicals
• Advanced primary care services including 24/7 provider access, data integration, risk stratification, and care coordination
• Acute care services for conditions such as allergies, strep, pneumonia, influenza, and skin conditions
• Disease management services for conditions such as diabetes, congestive heart failure, and asthma
• Occupational health services including drug and alcohol testing, pre-employment physicals, and injury care
• Well-being services such as tobacco cessation, biometrics, health risk assessments, and health coaching
