Prescription Dispensing Service

This contract is for prescription dispensing services provided by Uptown Drug to patients residing in the Kingman, Arizona vicinity. The services include dispensing Tuberculosis medicines on behalf of Mohave County Department of Public Health. The contract has a term of 2 years and can be renewed for up to 4 additional 12-month periods. The payment terms include a fee of $6.00 per dispensed Rx.
