Community Health Needs Assessment- Community Health Improvement Plan

The City of Allentown is seeking services to conduct a community health assessment and develop a community health improvement plan. The selected vendor will work with the Allentown Health Bureau (AHB) to gather and analyze data, create a multi-year plan, and establish a framework for re-implementing the plan every 3-5 years. The contract involves project management, staffing, and technical assistance. Key aspects of the contract include: • Conducting a community health assessment in collaboration with AHB staff • Providing project management and technical assistance to implement the community health needs assessment • Gathering and analyzing data to inform the creation of a community health improvement plan • Establishing a framework for re-implementing the plan every 3-5 years • Working with AHB and the City of Allentown to develop strategies to address the community health improvement plan
