Description

The County of San Mateo is seeking proposals to operate a Transitional Age Youth (TAY) Interim Housing Program and a TAY Rapid Re-housing Program. The programs aim to provide housing-focused case management, facility operation, food services, and linkages to clinical services to help TAY individuals transition to permanent housing. The Interim Housing Program will provide a 6-bedroom, 4-bathroom single-family home in unincorporated San Mateo County, while the Rapid Re-housing Program will provide housing identification assistance, time-limited flexible rental assistance, and case management services. • The programs will serve TAY individuals experiencing unsheltered homelessness in San Mateo County. • The goal of the housing plan is to address the client's current barriers and transition them to a more permanent housing situation within 150 days of program enrollment. • The County will provide maintenance services for the interim housing facility. • Both programs will require applicable reports, performance measure data, and invoices with supporting documentation.
