Description
Santa Fe County’s Community Services Department is seeking a qualified firm or licensed professionals to provide ongoing clinical guidance, interdisciplinary team development, and meeting facilitation for its county‑wide CONNECT Network of over 300 navigators across more than 70 service providers. The work includes designing a trauma‑informed care framework, delivering quarterly professional‑development sessions, preparing and summarizing meetings, and producing training materials and reports. Services must align with the Accountable Health Community (AHC) standard of care and comply with the County’s living‑wage ordinance. The contract begins in November 2026, lasts one year with the possibility of extension up to four years, and requires a minimum of 50 service hours per quarter (≈ 200 hours per year).
