Description
The Indian Health Service (IHS) Hopi Healthcare facility is issuing a Blanket Purchase Agreement (BPA) for the procurement of new dental prosthetic appliances (dentures) with a base period plus four optional years. The contract is a 100 % small‑business set‑aside and requires vendors to be original equipment manufacturers or authorized distributors, providing OEM warranty and service. All items must be shipped FOB destination to Hopi Healthcare in Polacca, AZ, with shipping costs included in the quote. Invoicing is electronic through the Federal Invoice Processing Platform (IPP). The award will be made to the lowest technically acceptable offer that meets all requirements.
