Description
The Colorado Department of Health Care Policy and Financing is seeking a contractor to provide Housing Navigation Services for Region 4, which includes Archuleta, Dolores, Hinsdale, La Plata, Mineral, Montezuma, Ouray, San Juan, and San Miguel counties. The contractor will be responsible for coordinating rental assistance vouchers, administering housing navigation services, and providing community outreach. The contractor will also be required to submit monthly reports and participate in evaluation meetings. The goal of the project is to ensure improved housing search and placement for individuals receiving Targeted Case Management-Transition Coordination. Some key requirements for the contractor include:
• Experience in locating and securing housing for low and/or fixed income individuals and/or families within Colorado
• Experience working with Medicaid Members and organizations that are a part of the disability and elderly community
• Ability to provide sufficient personnel to perform the work throughout the term of the contract
• Ability to use subcontractors, if necessary, to complete a portion of the work
The contract term is anticipated to begin on January 1, 2026, and end on December 31, 2026, with the possibility of extension. The maximum available amount for this project is $169,000.00.
