Department of Defense Pharmacy Uniform Formulary Blanket Purchase Agreement/ Uniform Formulary Additional Discount Program

The Department of Defense (DoD) is seeking quotes for pharmaceutical agents to be included in the Uniform Formulary (UF) and the Uniform Formulary Additional Discount Program (UF ADP). The DoD Pharmacy and Therapeutics (P&T) Committee will evaluate the quotes based on clinical effectiveness and cost effectiveness. The quotes should include prices for all NDC-11 packages of the pharmaceutical agent(s) and should be submitted by April 2, 2026. • The UF is a list of medications required to be on formulary at all full-service Military Treatment Facilities (MTFs). • The UF ADP is a program that provides refunds to the Government based on the accrued utilization of pharmaceutical agents. • The DoD P&T Committee will consider the relative clinical effectiveness and relative cost effectiveness of each class in recommending a cost share tier classification. • The quotes should be submitted electronically to the RFQ POC listed in Part 2.3 of the document.
