Description

The Dane County Department of Human Services is seeking proposals from qualified providers to operate a high-acuity community-based residential facility (CBRF) for adults with serious and persistent mental illness. The facility will provide residential support and structured services to individuals in need of high-acuity care. The provider will be expected to offer a range of services, including assessment, teaching, and practice of activities of daily living (ADLs) and instrumental activities of daily living (IADLs), as well as crisis de-escalation techniques and management of challenging behaviors. The provider will also be required to work with the Dane County Crisis Provider Network (DCPN) and to provide quarterly and annual reports on services provided. Some key points of this RFP include: • The provider must be able to operate a 5-6 bed co-ed home exclusively at the high-acuity level of care. • The provider must be able to assess, teach, and allow residents to practice their ADLs/IADLs. • The provider must be able to handle low-level nursing needs and provide medical observation. • The provider must be able to provide transportation to community integration opportunities. • The provider must be able to provide daily/weekly structure and offer residents the ability to participate in psychoeducational groups. • The provider must be able to ensure that staffing stays at a safe level and that the program can offer weekly outings for each resident without decreasing the staffing pattern at the home.
